Please review the updated flyer for our ANNUAL SPORTS PHYSICALS event on Monday, April 20th at 6:00 PM here at PJSHS. The most notable change is that the EKG's are not free, and the fee is $20. However, when registering online, parents may select “I need financial assistance” or “I do not choose to donate,” and the EKG will be provided at no cost.
Also included below is the FHSAA physical form(EL2), in which pages 1-3 and the top of page 4 will need to be filled out prior to being examined.
